Consumer Directed Services (CDS) Personal Care Assistance provides support and/or management with daily living tasks for individuals with physical disabilities.
This program allows clients to take control of their care by choosing their own caregivers. This service fosters independence and ensures that care aligns with individual preferences and needs.
Consumers are trained to hire and manage their own caregivers to assist them with their daily tasks.
CDS tasks may include:
- Personal Care (Bathing, Grooming)
- Mobility & Transfers (Assisting to/from wheelchair)
- Household Tasks (Cleaning, Laundry, Dishes)
- Advanced Personal Care Tasks (Ostomy/Catheter hygiene)
- Non- Medical Transportation (Shopping, Errands & Employment)
Consumer Directed Services (CDS) Program Eligibility Requirements:
- Must be at least 18 years of age
- Must be able to direct their own care
- Must be physically disabled
- Must require assistance with daily living tasks
Caregiver Eligibility for CDS Program:
- Be at least 18 years of age
- Be able to pass a Family Care Safety Registy screening
- Cannot be the consumer’s spouse
Horizon Home Care (HHC) provides on behalf of consumer the following:
- Collecting Electronic Visit Verification (EVV) records
- Payroll Processing and Employment Tax Functions
- Training and Orientation Program of CDS Employer Management Skills
