Healthy Children and Youth (HCY) Services provide medically necessary support for individuals with special health care needs from birth to 21 years of age. The resources from this program are designed to assist families to care for the child safely at home.
Services and Care may include:
- Personal Care (bath, dressing & grooming)
- Household Tasks Related to Child’s Needs (Laundry, Meal Prep)
- Advanced Personal Care (Ostomy Care, Transfer Devices)
- Nursing Visits
- Case Management
